Yutang Unified Diabetes Care is a cross-sector collaboration between iHealth, a healthcare tech company, and Tianjian Hospital of metabolic to explore a new healthcare service model for managing chronic disease. With the help of design thinking and new technologies, Yutang provides a solution for diabetes management that ranges from a simple and intuitive way of vital collection to automatic in-hospital exam data and at-home data consolidation, data analysis and finally convenient and timely communication with care team professionals.
